Orthopedic Specialists - Kauai
Catherine Shin, MD
3125 Akahi St. Lihue, HI 96766
Believe it or not, things are easiest for a medical office when a patient does not have insurance. The transaction is then similar to taking your dog to the vet, having your taxes done or getting your hair cut. We see you, determine what you need and charge you for what we do and the things we provide. You then pay using cash, check or credit card just you would normally pay the vet, your accountant or beautician. We are happy to see self pay patients who do not have health insurance. An office visit may cost you from $100 - $350 depending on if you need an injection, a cast or to be stitched up.
When an insured patient sees a physician, the physician can either be "in network" or "out of network" with each insurance company. Depending on whether the physician is in or out of network will affect how much you have to pay and how much the insurance company pays. Some insurance plans have office copays for visits and some have deductibles with a coinsurance percentage. The co-pay or co-insurance that you pay at the office visit can be anywhere from nothing to $40, depending on your plan. ORTHOPEDIC SPECIALISTS KAUAI PAYMENT POLICY Orthopedic Specialists Kauai requires a copy of your insurance card and your driver’s license or other form of ID in order to file your claim. You can request your insurance company to fax or provide you with documentation of insurance coverage that includes all billing information. You will need to provide this information every time you change insurance companies, as we will not be responsible for any denied claims if correct and up to date information is not given at the time of service. For the most up to date information regarding which plans we are contracted with, please contact Lori Fitzgerald at fitz.lori@gmail.com or (808) 632-0200. ALL CO-PAYMENTS ARE DUE AT THE TIME OF SERVICE. We will collect your co-payment at the conclusion of your visit. If you are not prepared to pay your co-pay today via cash, check or credit card, please reschedule your appointment at the front desk now. In addition, any check that is not honored by your bank will incur at $10.00 service fee plus any fees charged by the bank. If you do not have insurance coverage, we request that payment be made at the time of your visit. If you are unable to pay the entire balance and it exceeds $200.00, we will accept a partial payment on your first visit of $200.00 and set up arrangements for the balance. You may sign a payment plan and make payments within each 30-day billing period until the balance is paid in full. Service charges will automatically be added to any account over 90-days past due, but those charges will automatically be written off as long as you remain current on your payments. This same policy applies to the balance of insured patients after their insurance has paid their portion. If you need surgery, we will preauthorize your procedure, let you know what your benefits are, and what your approximate balance will be. Insured patients will need to make their co-payment or a $50 scheduling fee (whichever is less). Self-pay patients must make a pre-payment of $200 BEFORE surgery can be scheduled, and you will have to sign a written payment agreement for the balance.

As a courtesy to our patients, Orthopedic Specialists Kauai has enrolled with several insurance companies. In doing this, we agree to file your insurance claims and accept the contracted discount rates from your insurance company. We recommend that you always verify your coverage with your insurance company and verify (if not already known) that we are covered by your particular plan prior to scheduling a visit or surgery. You will be responsible for charges that your insurance company does not cover.
